Target Wellness Grants


Regular Target Wellness Grants

Target Wellness grants are for projects up to and including $15,000 which improve the health and wellness of people living in the Mat-Su Borough. We accept Target Wellness grant requests continuously throughout the year. All applications should be submitted 45-60 days prior to your project or program to allow adequate time for review. Before applying, please review our grant guidelines for information about what the Mat-Su Health Foundation does and does not fund.

Target Wellness Earthquake Recovery Grants

NEW: The Mat-Su Health Foundation is offering Target Wellness earthquake recovery grants to assist qualified Mat-Su nonprofits affected by the November 30 earthquake. The application process is the same for regular Target Wellness grants, but these special grants will receive expedited handling, and the maximum amount will be $50,000 instead of the usual $15,000 limit on Target Wellness grants.

What we will not fund

  • Goods or services provided prior to Nov. 30 earthquake
  • Expenses without documentation or invoices
  • Replacement of personal items damaged in earthquake
  • Regular operating costs of organization (personnel or expenses) not related to lost revenue due to closure
  • Repairs/remediation costs that will be covered and/or reimbursed by insurance unless claims adjustment will create critical, ongoing program delays

Target Wellness earthquake recovery may retroactively fund needs associated with the November 30 earthquake and projected needs based on estimates or other documentation.

Examples of items that could be funded by Target Wellness earthquake recovery grants include the following:

  • Clean up crews
  • Overtime wages associated with clean up
  • Damage assessments
  • Inspectors for gas/water/other services
  • Plumbing repairs
  • Lost revenue due to closure from earthquake
  • Water remediation
  • Replacement cost of items damaged in earthquake
  • Repairs
  • Materials for repairs
  • Insurance deductibles
  • Extra utility cost for water system
  • Support to build and develop coping strategies for students in affected schools
  • Support for affected staff including coping strategies and/or therapeutic options
  • Technical assistance associated with earthquake recovery

Other items can be presented for review. Organizations are encouraged to document damage with as much detail as possible, including photographs, for supporting documentation. How to Apply for Target Wellness Grants

Grant applications must be submitted online at this link.

You will need the following information to complete the application:

  1. Organization's Tax I. D. Number
  2. Organization's Legal Name and Address
  3. Organization Also Known As (if applicable).
  4. Phone, Fax, Email Address, Website address
  5. Organization Type, i.e., nonprofit, city government, etc.
  6. Organization's Primary Contact Information
  7. Project Title, Request Amount, Project Budget, Term
  8. Project Start Date and End Date
  9. Project Description, Statement of Need
  10. Target Population, Community Support
  11. Sources of Revenue
  12. Acknowledgement: Describe how you will generate positive public relations for Mat-Su Health Foundation if you are awarded this grant.
  13. Project Budget: Please provide a project budget
