The goal of the IMPACT program is to provide effective and efficient treatment for depression and anxiety in the primary care setting by using a collaborative care model that includes the primary care provider, a care manager and consulting psychiatrist. While still above the targets, clinical outcomes have declined slightly in the past six months and are expected to improve as the issue is resolved this autumn.GRANT SUMMARYThe goal of the IMPACT program is to provide effective and efficient treatment for depression and anxiety in the primary care setting by using a collaborative care model that includes the primary care provider, a care manager and consulting psychiatrist. Patients are screened for depression at each visit and individuals who screen positive are assessed by the provider. Patients who screen positive for depression are referred into the program and entered into a registry. The registry is used to track outreach to these patients and document response to treatment as measured by the PHQ-9 and GAD-7 screening tools. The screening tool scores provide measurable clinical data that is used to help determine the effectiveness of treatment. A drop of 5 points or greater in PHQ-9 score from baseline indicates that the patient has an improvement of symptoms and a drop of 50% from baseline or a score under 10 (out of a possible 27) after at least 10 weeks of treatment indicates that the patient may have achieved remission and is ready to move out of active treatment. At the end of this reporting period patients in active treatment were, in the aggregate, at or above the clinical outcome targets for this model.48% of patients in active treatment have at least a 5 point decrease in their PHQ-9 score (target = 40%)67% of patients in active treatment have at least a 50% decrease in PHQ-9 score, or a score under 10 after at least 10 weeks of treatment.64% of patients in active treatment have at least one psychiatric consultation note in their chart.DESCRIBE HOW GRANT FUNDS HAVE BEEN UTILIZED TO DATE.Grant funds for this project pay the salaries for the care manager and project lead, and for community outreach and education.PLEASE INCLUDE ANY ADDITIONAL INFLUENCING FACTORS THAT IMPACTED YOUR OUTCOME RESULTS TO DATE (INCLUDE BOTH POSITIVE AND NEGATIVE).None
