Pilot Project: REASON — Residence-Based End of Life and Aging Services of the Northern Valley project. Pilot a palliative care program for older adults in need and those at end of life, to enable them to age in place in this rural/remote area. The program will help fund a chronic care management program, launch a community volunteer program, and increase awareness around palliative care.
