Target Wellness grants provide up to $25,000 for projects dedicated to improving the health and wellness of people living in the Mat-Su Borough.
Eligibility and requirements
- Grant Amount: Up to $25,000
- Deadline: Applications are accepted year-round
- Eligibility: Non-profit organizations offering services or benefits to Mat-Su residents. Please review our grant guidelines for full details.
- If you are working collaboratively with multiple organizations on a project, please submit just one grant application for the project.

How to Apply
Gather Materials
We recommend gathering this information before starting the application.
- Organization’s Contact Information
- Organization’s Tax I.D. Number and Legal Name
- Project Title, Request Amount, Project Budget
- Project Start Date and End Date
- Project Description, Statement of Need
- Target Population, Community Support
- Current Revenue Sources
Submit the Grant Application
When you are ready to apply, access the grantee portal to complete and submit your application online.
After starting the application, you can pause and finish it later, if needed.
If you would like to forward copies of your application to coworkers for review, you can do so by sharing the link to your account.
Notification
You will be notified of your funding decision according to the timeline outlined below.
You can access a copy of all grants submitted by logging into the grantee portal at any time.
Target Wellness Grant Application Timeline
The total decision-making timeline, from application receipt to payment processing, takes between 85 and 105 days (approximately 3 to 4.5 months).
Intake: 1-2 days
Immediately following receipt of your application, the foundation will confirm your 501(c)3 status and assign your application for further review by our review team.
Review: Up to 45 days
During the review process, our team will read your application materials, confirm your eligibility for the grant selected, and draft a decision recommendation.
During this time, we may also contact your organization with additional questions.
Decision: Between 5-7 days
Once a decision recommendation has been drafted by our review team, your application will undergo a final decision review.
Processing: Between 5-7 days
If approved for funding, we will begin processing a grant agreement, establishing grant requirements, and initiating grant distribution
Payment: Between 30-45 days
Once initial processing is complete, we will send your payment request to our accounts payable vendor, Stampli, for payment processing and delivery.

Progress Report Form
All grantees are required to submit annual progress reports on their grant projects. To submit a progress report, log into your account and click on the “Requirements” tab to access the form. Once completed, click “Review and Submit” to submit the form.
Financial Statement Information
Prospective grantees applying for funds from the Mat-Su Health Foundation will be required to submit financial statement information about their organization, as well as their project. You can review this sample statement to understand what information is needed.
